Care Management

Our unit actually works within the acute care units highly specialized in the care of musculosketal disorders with a focus on surgical orthopaedic and spine patients. We are involved, with our Social Work colleagues, in supporting and guiding the patient from pre-admission to discharge in obtainining the required information and services.
Our Facilities
Case managers work in partnering relationships with Social Workers on unit level post-discharge planning needs of assigned patients. The Case Manager ensures the availability of required post-discharge medications and services. They also serve as the interface between providers and insurers in verifying the need for acute care services.
Staff Quality
- Strong assessment skills with a clinical background in orthopaedics and spine. Ability to interact effectively and to advocate with internal and external care providers, including insurers, to meet patient needs
- Flexibiltiy in adapting to changing circumstances with the ability to quickly analyze and respond to new situations
- Skilled in interdisciplinary collaboration; team player

Previous Discharge planning/ Case management and/or Utilization Review experience prefered. 3-5 years clinical experience in acute care required
